Improving Diagnosis of Pneumoperitoneum with AI

Understanding the Issue

Delays in diagnosing pneumoperitoneum, which is air in the abdominal cavity, can seriously affect patient survival. Most cases in adults are due to a perforated organ, often requiring surgery. Although CT scans are the best diagnostic tool due to their accuracy, there are frequent delays in busy emergency rooms and differences in interpretation among medical staff.

AI Solution: PACT-3D

Researchers developed an AI model called PACT-3D to quickly identify pneumoperitoneum in CT images. This model was trained on a large dataset and validated with high sensitivity (81-83%) and specificity (97-99%). When minimal air cases were excluded, sensitivity improved to 92-98%. PACT-3D uses advanced 3D imaging techniques to provide accurate, detailed results.

Data and Methodology

The research followed strict guidelines and was approved by relevant ethical boards. The model was tested on a variety of CT scans from 2012 to 2021, which were carefully reviewed by radiologists. A state-of-the-art 3D U-Net model was used for precise location detection, ensuring that even small amounts of free air can be identified.

Model Performance

Analyzing 139,781 scans showed the model’s effectiveness, with an F1-score of 0.54 in simulated tests and 0.58 in real-world applications. In external testing, it achieved an F1-score of 0.80, with better sensitivity for larger volumes of air. The model’s architecture allows it to distinguish between free air and bowel gas, which is crucial for emergency cases.

PACT-3D demonstrates the potential to enhance diagnostic efficiency in emergency care, which can lead to better patient outcomes. While it is effective, further refinements are needed to improve its sensitivity for detecting smaller air volumes.
